隨著科技的進步,新型態的概念運算模式雲端運算(Cloud computing)促進各種產業的技術蓬勃發展。近年台灣衛生福利部所推行的「電子病歷」、「健保雲端藥歷系統」、「健康存摺」等相關延伸計畫的成功,都是最好的例子。不過仍尚欠缺完整的醫療雲端服務架構中,病患共同參與病歷製作的行為。台灣社會中,偏頭痛及其相關疾病之影響年齡層相當廣泛,其中人數比例又以青壯年人口(25歲至45歲)最高,因此偏頭痛之相關研究十分受到台灣醫療領域的重視。台北榮民總醫院神經內科的頭痛門診治療流程,主要分為初診問卷調查、醫師初步診斷、頭痛日記追蹤及物理藥物治療。從過去數十年至今,台灣許多醫院依舊使用傳統紙本方式記錄門診的問卷及病患相關資料的追蹤存檔。而整個偏頭痛的治療時程會依病患情況並不固定,最長可達數十年的控制與治療,因此不論是資料存放或是後續相關資料之研究分析,都十分不便,醫師若要針對特定條件的資料分析,必須先透過醫護人員或助理行政人員將紙本紀錄進行量表分數計算後打字建檔,再匯出透過現成套裝軟體進行數據分析。傳統紙本手寫方法,有著資源的消耗、耗時的行政作業以及資料保存管理的缺陷;其中包括人為疏失造成資料填寫錯誤或遺失、資料本身因環境因素損壞等。因此,本研究目的為建置一套符合醫療產業使用者需求的系統架構及流程,並進一部開發出符合現代使用者習慣模式的雲端臨床醫療輔助整合系統,以新型態架構及流程,將各終端使用者輸入的資料與主治醫師的操作,及相關醫療人員研究之作業流程進行全面整合,即時提供醫師在門診中所需的所有資訊。藉此改善對於病患、主治醫師或是相關醫護人員的作業效率,並提升整體醫療品質及效率,且透過無紙資訊化,達到友善環境之福祉。
英文摘要
In recent years, with advanced technology, a new pattern Cloud Computing is de-veloping prosperously and bringing the advancement of technology in practically every profession. In Taiwan, the Electronic medical record (EMR), Pharma Cloud System, My Health Bank, and another related cloud system which engaged by Ministry of Health and Welfare are the best proof. However, it still lacking the involvement of pa-tients to complete the fifth level of Electronic Health Record(EHR). Nowadays, in modern Taiwan society, the effect range of a migraine and other related diseases are very extensive. Among all the range, the percentage of the young and middle-aged population (from 25 to 45 years old) is the highest. Therefore, the migraine-related re-search of are wildly popular and has been highly regarded in Taiwanese medical field. The Basic flow of migraine treatment can be dividing into four parts; survey of the initial registration form, diagnosis, the track of headache diary, and physical therapy. Since the last decade, many hospitals in Taiwan still using paper to record the initial registration form, headache diary and other patient related information. The whole treatment process depended on each patient and thus the time of therapy can probably take decades to be finished. Hence, it will be challenging and inconvenient no matter the data storage or future related data analysis. Furthermore, before the attending physi-cian attempt to analyze particular relations or attributes, the administrative assistant or healthcare professional have to input all the paper data into existing Clinical Study In-formation System (CSIS). Traditional handwriting method causes the wasting of paper, time-consuming of administrative processes, and data storage difficulty, which includes human error inaccuracy, physical damage and loss of data. The primary purpose of this study was to construct a cloud integrated system, which provides a modern and optimal viewing and interaction experience for any users. By using new structure and flow, this platform can fully integrate the usage of all physician, healthcare professional, and patient. To be more important the platform can provide a real-time data support for doctors, improving the quality of health care and the change of paperless can be eco-friendly.
